To play Powerball Lotto using this form, follow these steps:

  1. All players must be at least 18 years of age!
  2. Choose how you want your numbers selected. Do you want your numbers chosen at random from a State Powerball Lotto Machine (i.e. Quick Pick)? Or, do you want to choose your own numbers?
  3. Please tell us how many games you wish to play?
  4. Please tell us how many times you want us to play your numbers (number of draws).
  5. Please select how you want the jackpot paid.
  6. Please multiply the number of games by $2.00 (Minimum $20.00) and put the total amount in the field marked (Total=).
  7. Orders for this form must be in by 2:00 pm (EST)   Wed. and Fri.

When choosing your own numbers, enter your selected numbers between 1 and 55 plus 1 additional number between 1 and 42 for each game you wish to play. There is room for up to 20 games.

Minimum order is $20.00 for each Lotto Game. For a cost of $20.00 you may play either a ONE DAY play (10 games for one draw only) or a subscription one game minimum 10 draws (New York and Illinois are 20 games for $20.00 ONE DAY or 2 games 10 draws $20.00)
